Blackrock Blues is the first mission in Episode 4: Fury, then Silence. In this episode, Will has to get medical supplies to help heal Franklin after Mathis beat him up.

When Will awakens, he will be in a cell, and hear the voice of Franklin. Stand on the toilet to talk to him and learn more about Blackrock.
Following the cutscene, the player will want to leave the prison and search the vehicles and buildings around the prison yard. These will yield a number of useful items, such as granola bars, flares, a prybar, fuel for fires, and low-tier clothing. One of the buses contains a note which will start the Dark Star quest. If the player gets cold while searching, it's possible to stand next to the fire barrel by the prison doors, which doesn't go out.
Follow the signs to get to the infirmary. Once there, try the front door. It will be locked, so the player will have to go out of the prison yard via the two doors left open and enter the small forested area north of the prison. There are a number of wolves here, which can be dealt with using flares, torches, or stones. If passing close to the cliffs, it's possible to bypass many of the wolves and pickup the key from a dead guard, which will start the Blackrock Lockers side quest.
Re-enter through another gate (left open) in the fence, and pass through the guard accommodations. Searching these will yield some food and clothing. When finished searching, climb upstarts and leave the accommodations. A catwalk will lead to the Infirmary. Once inside the infirmary, Jace will call. Answer the phone and learn about her role in keeping Donner in solitary. Searching the infirmary will yield a note telling the player where to find the key to the front door of the prison. After leaving the infirmary, cross the courtyard you originally started in to find the exit along the wall to the left.
Shortly before exiting, the player will come to a weapons locker. This can provide up to two noisemakers, a Distress Pistol, and a few Flare Shells, depending on the difficulty. Once outside the prison, head along the main road in a generally southward direction, then turn east and cross an old log bridge. Across this bridge, a large patch of blood-stained snow will be visible near two pickup trucks. Follow this track to the south for a bit, then turn east towards a cave. The required medicine will be on the body of the dead prison guard.
With the medicine in hand, follow the main road back to the prison. Before dropping off the medicine, place any items found along the way in a safe location (the locker in the guardhouse is recommended), else they'll be confiscated. The mission ends when the drugs are delivered at the jail's door.

- The name of this mission may have been inspired by "Folsom Prison Blues" a country song about prison life written by Johnny Cash.
